Since the first successful egg donation pregnancies in the 1980s, the egg donation procedure has become well-established and widely practiced, both globally and here in South Africa.

Possible risks of donating eggs
That said, egg donation involves medical treatments and a minor medical procedure, and therefore it does carry some risks. Read more below about the possible risks.
Hormonal medication
As a donor, you’ll take hormone injections to encourage your ovaries to produce multiple mature eggs. This is the most significant part of the process, and it is closely monitored by fertility specialists.
The main risk is a rare condition known as Ovarian Hyper-Stimulation Syndrome (OHSS), where the ovaries become overly responsive and enlarged, sometimes causing abdominal discomfort or bloating. This condition only occurs in a very small number of cases – around 1% of IVF cycles.
Most donors do not experience any side effects, though some report PMS-like symptoms such as mood swings or mild discomfort.
To ensure your safety, frequent ultrasounds and check-ins are conducted throughout your cycle to track your response to the medication.
The Retrieval Procedure
Egg retrieval is performed using mild general anaesthetic and ultrasound guidance. While under sedation, your fertility specialist will use a fine needle to collect the eggs from your ovaries. The procedure typically lasts about 30 minutes and involves no cutting or stitching.
After the procedure, you’ll recover for about an hour at the clinic. Mild cramping or period-like discomfort may follow, but this usually resolves quickly with over-the-counter pain medication. You’ll need to take the day off for the procedure and arrange for someone to drive you home. Many donors choose to rest the following day too.
Although egg donation is considered very safe, as with any medical procedure, there is a small risk of infection, bleeding, or anaesthetic-related side effects. Your fertility specialist will discuss these with you in detail.

All egg donations are anonymous and confidential. Donors and recipients will never know each other’s identities or details. At Cape Fertility, your identity is never shared.
Importantly, any child born through egg donation is legally and biologically the child of the recipient mother. Although your donated egg contributes to conception, the recipient’s body nurtures the baby during pregnancy, making her the baby’s biological and legal parent.
How Egg Donation Affects Your Fertility
Women are born with around two million eggs, and each month, a number are naturally discarded if not fertilised. During a donation, 10–20 eggs are retrieved – eggs that would otherwise have gone unused.
There is no scientific evidence that egg donation impacts your future fertility. Many donors go on to have children of their own when they choose to.

When you are selected by a couple to donate, we’ll arrange and pay for your travel and accommodation. The process comes at no cost to you.
You’ll also receive compensation as allowed under South African law – currently R9000 per donation cycle at Cape Fertility – to cover your time and expenses. This payment is made to you directly after your donation, and you’ll receive the same compensation for every cycle if you choose to donate more than once.
